Clean claim rate, first-pass rate, denial rate and days in AR
You do not need a big dashboard. Four numbers show whether your billing is healthy.
Clean claim rate
The share of claims that are error-free when you send them. A low rate means rework. Count claims with no errors at submission, then divide by all claims submitted in the same period.
First-pass rate
The share of claims paid on the first submission without being denied or resubmitted. It shows how well your front-end checks and coding work.
Denial rate
Denied claims divided by claims submitted. Track it by payer and by reason, because the pattern is more useful than the total. See why claims get denied.
Days in AR
A measure of how long, on average, it takes to be paid. Your aging report shows where older claims are building up. Our guide to reading an AR aging report explains the buckets.
How to use them
Write the four numbers down every month in the same place. Do not chase a perfect target. Look for direction: are they getting better, worse or staying flat? If they are getting worse, talk to us.
