Denial management and appeals for independent practices
A denial is not the end of a claim. We find out why it happened, fix it, appeal with the right documents and show you how to stop the same denial coming back.
What is denial management?
Denial management is the process of tracking claims that insurers refuse to pay, working out the cause and acting before the appeal deadline passes. Good denial management does two things: it recovers the money from today’s denials and it reduces tomorrow’s denials by fixing the cause.
Common reasons claims are denied
- Inactive coverage or wrong insurance details at the time of the visit.
- Missing or expired prior authorization.
- Coding, modifier or diagnosis errors.
- Claims filed after the payer’s time limit.
- Duplicate claim submissions.
- Missing documents or a medical necessity review.
Each reason needs a different fix. A single “resubmit” habit does not work for all of them.
How we work your denials
- Capture the denial and its reason codes from the remittance advice.
- Sort by amount, age, payer and the deadline for appeal, so the urgent ones go first.
- Correct the claim when the error is ours to fix, or appeal with the required documents and a clear letter.
- Follow up with the payer until the claim is reprocessed and paid, or a final answer is recorded.
- Report the patterns: which payers, codes or workflows cause repeat denials.
What you get
A weekly report with the denial rate, the top denial reasons and the status of every open appeal. When the cause is in the front office, such as eligibility checks or authorizations, we tell you exactly what to change.
Where this fits
Denial work goes hand in hand with AR follow-up and prior authorization and eligibility checks, because many denials begin before the claim is sent. We can take on one service or the whole chain.
Frequently asked questions
What does a denial management company do?
It tracks denied claims, finds the cause, corrects or appeals them before the deadline and reports patterns so the practice can prevent the same denial in the future.
How long do I have to appeal a denied claim?
The limit is set by each payer and plan, and it can be short. That is why we sort denials by appeal deadline first and record the date for every claim.
Do you only fix denials, or also prevent them?
Both. Recovering current denials is the first job. Then we share the top causes in your weekly report, so eligibility, authorization, coding and documentation problems can be corrected at the source.
Do you guarantee that appeals will be won?
No. Outcomes depend on the payer, the documents and the clinical facts. We guarantee careful work, timely filing and clear reporting.
Do you handle commercial and government payers?
We work denials according to each payer’s rules. We confirm the payer mix and the portals needed during the discovery call and the baseline audit.
Arovia RCM does not guarantee collection outcomes. Please do not send patient information through the website or by email before agreements are signed.
