How to appeal a denied medical claim, step by step
Many denials can be corrected or appealed, but only if you act before the payer's deadline.
1. Read the denial carefully
Find the reason and remark codes on the remittance advice, and note the appeal deadline. Deadlines differ by payer, so check the payer's policy for the exact time limit.
2. Decide: correct, resubmit or appeal
If the denial came from a simple error such as a wrong ID or missing modifier, a corrected claim may be faster than a formal appeal. If the payer disagrees with the service itself, you will usually need a written appeal.
3. Gather the evidence
- The original claim and the denial notice.
- Clinical notes that support the service.
- Authorization or referral records, if they apply.
- Any payer policy that supports your position.
4. Write a short, specific appeal
State the claim details, the reason for denial and why it should be paid, and attach the evidence. Keep it factual and clear.
5. Send it and track it
Use the payer's required method, keep proof of submission and set a reminder to follow up. Record the outcome so you can see patterns across payers.
